1) Click here to download credit card authorization form and return via scan/email to Colter. The credit card will be used for the set-up fee, as well as any applicable monthly storage charges.

2) Click here to download W9 Form and return via scan/email to Colter.

3) Click here to download Agreement/Payout Preference Form and return via scan/email to Colter.

4) Individual Product Details. Please provide the following info for EACH product included in the store:
a) Name
b) Price
c) Description (optional but helpful for your customers)
d) Square Image with white or transparent background- 600×600 px or greater
e) List of available product variants. (sizes, colors, autographed, digital download, etc.)
f) Quantity of each variant
g) UPC codes for media (CD, DVD)

5) Graphics for the store page:
a) Artist banner image with name/logo 1200X430 pixels
b) Artist Logo image 148X148 pixels
* Images may be either .jp or .png, 300ppi

6) Starting inventory of physical products.

Note there is a $10.00 charge per carton for items received without a packing slip. We recommend sending an estimated 90-day supply of product. Contact Colter if you need help determining the best starting inventory. Of course any products manufactured and printed by Bandwear will already be in our warehouse for inventory.
